What is Home Hospice Care?
Home Hospice is a philosophy of care provided to patients and their families wherever they call home. Hospice care enables individuals with life-limiting illnesses to live life with quality, dignity and comfort.
Signs it may be time to consider Hospice Care
Many people miss opportunities for comfort by waiting too long to consider hospice care. Signs that might indicate a person may benefit from hospice services include:
Frequent visits to the Emergency Room
Shortness of breath
Unplanned weight loss
Frequent falls
Sleeping or resting more than usual
Diagnosis of life-limiting illness such as cancer or Alzheimer’s Disease
Home Hospice Care of CNS
As Utah’s first Home Hospice Care agency to be certified by Medicare to offer Hospice Services, CNS has developed a legacy of trust and compassionate service. We emphasize creating moments and experiences that provide peace of mind to our patients and their families. Our Hospice care professionals put health, dignity, comfort, and well-being first.
What does Hospice Care include?
Home visits from physicians, nurses, home health care aides and volunteer companions
In-home counseling and assessment visits as needed from social workers
Access to spiritual counselors
All medications related to the terminal diagnosis and pain management
All home care medical equipment such as hospital bed, wheelchair and shower safety equipment
Life Stories - a CNS volunteer brings recording equipment to capture your loved one's stories. The family receives a keepsake DVD of the recording.

Special programs for CNS Hospice patients
The Senior Wish Program grants personalized wishes to our Hospice patients. The wishes provide the opportunity for our patients to relive a cherished memory or experience a new adventure.
The Honor Salute Program is for our Hospice patients. It gives pride and peace to our veterans while bringing together family and friends to celebrate what is often the final time these men and women are publicly thanked for their service. Veteran patients are saluted, presented with a certificate of appreciation and pinned with a veterans' appreciation pin.
